A padlock is installed through the lockout to ensure it isn't removed by unauthorized personnel. These devices are an integral part of a facility lockout/tagout (LOTO) procedure designed to meet the OSHA standard for the Control of Hazardous Energy (Lockout/Tagout).

Permanent-mount ball-valve lockouts permanently attach to pipe next to a ball valve to prevent valve handles from being turned. A hook feeds through the lockout and holds the handle in place.
