Answer: 
Grainger has contacted the product supplier and has been advised by them of the following.

The round damper is a flap that fits inside the duct connector fitting and opens outward when the fan is turned on. It only opens to the outside to prevent air from coming back into the room being vented. The fan can be mounted on a wall but still needs a 4” duct to run from the fan to the outside.
