NPP's FireRescue and PublicSafety GPO Contract #409 Grainger Contract Participation Form

To utilize the Master Purchase Agreement #409 for Maintenance, Repair and Operating Related Supplies, please review and complete the required information below and return for enrollment into the agreement. Master Purchase Agreement #409 for Maintenance, Repair and Operating Related Supplies ("Agreement"), between National Purchasing Partners, also d.b.a., FireRescue GPO, ("FireRescue GPO"), PublicSafety GPO, ("PublicSafety GPO”) and W.W. Grainger, Inc. ("Grainger"), is made available to all active and participating NPP, FireRescue GPO and PublicSafety GPO Members ("Participating Members"). Members shall complete this Grainger Contract Participation Form to affiliate to this Agreement and receive the benefits of the Agreement.

The Participating Member acknowledges and understands the following:

• Member may elect to participate in only one (1) Grainger catalog-wide agreement at any given time; and
• This Contract Participation applies to all Member accounts with Grainger; and
• Agrees to purchase MRO equipment and supplies in accordance with Master Purchase Agreement #409; and
• This Contract Participation Form and the above referenced Agreement supersedes all previous declarations for the purchase of product from Grainger; and
• The Participating Member further understands and agrees that enrollment will remain in effect during the contract term including any contract extensions, unless otherwise instructed by Member.
